Who We Are:
The Psychiatric Rehabilitation Services program is seeking a Case Manager to join our team. The ideal candidate for this position is goal orientated in optimizing the quality of life for the population we service by promoting recovery, independence, psychiatric stability, and community integration opportunities. This role may require working and maintaining some primary job duties in a telework environment.
 
Minimum Education and Experience:
Successful candidate will possess a bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Experience in targeted case management with adults with serious mental illness in a CSB setting is preferred. A QMHP-A certification and master’s degree in the Human Services or related field is preferred.
 
Required Knowledge, Skills, and Abilities:
Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers.
 
Proficiency in documentation writing of consumer services, time management and problem-solving skills preferred.
 
Additional Requirements:
Current valid driver’s license and good driving record required. Based on the Virginia DMV point system, record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.

Pre-employment drug testing, FBI criminal background check and education/degree verification required.

Duties include but are not limited to the following:
 • Assesses individual's strengths and needs and developing and implementing individualized service plans (ISP).
 • Implements treatment interventions and techniques to support individuals in problem-solving and goal setting.
 • Coordinating referrals for behavioral and physical health services, monitoring behavioral and physical health, and facilitating communication and coordination of care with individuals and their healthcare providers.
 • Meets with individuals face-to- face in the community and their homes.
 • Links individuals with other community resources.
 • Provides support to help individuals obtain and maintain housing including on-time payment of rent and household maintenance.
 • Educates clients and their families on their legal rights and provides information on department policies, procedures, and services.
 • Performs other related duties as required.
