Habilitation Coordinator

This position is responsible for providing services to adult consumers with an Intellectual or Developmental Disability residing in a Nursing Facility.  The duties include, but are not limited to, contact with consumers, families and/or legally authorized representatives to establish a positive working relationship, determine whether an individual identified as having mental illness, intellectual disability, or developmental disability needs nursing facility services, to identify alternate placement options when applicable; and to identify specialized services that may benefit the individual.  Position will complete the PASRR process upon timelines for contract requirements. This position requires the ability to identify service needs, work under pressure and stressful situations in a professional manner, be thorough in assessment and documentation, identity a variety of community resources to meet the service needs of each individual, negotiate and advocate with the service units for each individual, and develop a positive relationship with both the Nursing Facility staff, the individuals served, LAR’s, and service providers.  These duties are performed under supervision and in accordance with established policies with some latitude for independent judgment.  This position will work cooperatively with service providers in the community, always ensuring the consumer’s rights are respected and observed.  Skills in written and oral communication, problem solving and teamwork are essential. The may require the use a personal vehicle for performing daily activities.  This position reports directly to the IDD Intake & Continuity of Care Program Administrator.

QUALIFICATIONS:

Must have a bachelor's or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice; and must have at least one year of experience working directly with individuals with intellectual or other developmental disabilities. Must possess a current Texas driver’s license and qualify for Center insurance coverage.  Must be proficient in computer applications such as Word, Excel, Outlook, etc.

 

Salary Range:$19.14 To 20.55 Hourly
