Case Manager/ Service Facilitator

If you, or someone you know would like to help families find Answers, Help and Hope by becoming a Service Facilitator (Case Manager), we have openings across the state of Virginia
Successful Service Facilitators will have a minimum of an Associate's degree in a human services field or a Bachelor's degree in any field with 3 years experience working with or helping (in the case of a family member or friend) individuals with disabilities; they will have excellent time management/organizational skills, a strong working knowledge of Google Suite, comfortable working with technology, strong/stable internet and a reliable vehicle to visit our client's home. The majority of client visits are on a 60 day rotation.
