Intensive Care Coordinator

Position Description: Intensive Care Coordination includes facilitating necessary services provided to a youth and his/her family designed for the specific purpose of maintaining the youth in, or transitioning the youth to, a family-based or community-based setting. Intensive Care Coordination Services are characterized by activities that extend beyond regular case management services that are within the normal scope of responsibilities of the public child serving systems and that are beyond the scope of services defined by the Department of Medical Assistance Services as “Mental Health Case Management.” Clients eligible for Intensive Care Coordination include youth already placed in out-of-home care or youth at risk for out-of-home placement. This position involves both clinical and case management skills in working with children, adolescents, and family members.        

Qualifications: Bachelor’s degree in human services related field plus one year of clinical experience working with children/adolescents in a mental health setting required. Must be registered with the Virginia Department of Health Professions as Qualified Mental Health Professional-Child (QMHP-C) or Qualified Mental Health Professional Trainee (QMHP Trainee). Must be willing provide services consistent with the High-Fidelity Wraparound (HFW) model in which the incumbent will be trained; will be required to receive weekly supervision from a licensed mental health provider who is also trained in the HFW model. Must possess a valid VA driver’s license and maintain a safe driving record according to RACS policies.
