Case Manager

WHAT YOU WILL DO:
The Case Manager will be primarily responsible for the child and accountable for the coordination and provision of quality services to ensure the child’s reunification with family or qualified sponsors. The Case Manager will maintain an active caseload of no more than 15 enrolled children and provide intensive case management for medical exams, educational assessments, legal services and the child’s family reunification. The Case Manager will prepare assessments and any related social or medical history reports or forms as they pertain to the child's needs and case record. Establish a relationship with the child. Participate in child staffing and follow-up as assigned. Coordinate the service planning process. Must ensure concise and timely completion of all paperwork or documentation, including but not limited to, administrative, clinical or caseload requirements. Ensure and safeguard the human and legal rights of children. Provide quality services and maintain desired productivity level as defined by the supervisor. Comply with all policies and procedures as outlined by funding sources and state licensing requirements.

WHAT YOU WILL NEED:
  • BS/BA degree in the human services field.
  • At least 1 years’ experience working with children.
  • Experience in a residential, child welfare or refugee setting, preferred.
  • Bilingual communication skills in Spanish /English, required.
  • Proof of Immunization for: Varicella (2 doses), MMR (2 dose), Influenza (1 dose yearly), Tdap (1 dose)
  • Must have a valid driver license and auto insurance.
