Behavioral Health Case Manager - School Based

Case managers work with children and adults in the home, school, and community using a Trauma Competent framework. Services address the individualized mental health needs of the client. They are directed towards adults, children, adolescents and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual. The purpose/intent of case management is to provide specific, measurable, and individualized services to each person served. Case management services focus on the individual's ability to succeed in the community; to identify and access needed services, and to support the client in showing improvement in school, work and family and integration and contributions within the community.
The successful applicant for this position should have a bachelor's degree in related social services field; experience and aptitude in working with children and families in crisis situations, knowledge of working with youth in adoptive placements, knowledge of working with at risk youth, a demonstrated ability to work well with many different types of people; possess a valid driver's license, a good driving record, the ability to drive to community locations, automotive liability insurance, and a car which is safely maintained. Demonstrates knowledge of working with multiple systems as well as birth, adoptive and foster families.
