This new Mobile Response and Stabilization Service will provide a mobile, face-to-face response for a family-defined crisis involving a child or adolescent to avert unnecessary hospital Emergency Department visits and to reduce the likelihood of a crisis developing. MRSS provides needed support and intervention before emotional and behavioral issues escalate.

General Summary:
The Mobile Response Clinician will be deployed to support families and children within Rutland County when support is requested. This position will provide onsite support through positive behavioral support, WRAP around approach, evidence-based practices…etc. Supports will be deployed on an as needed bases with additional follow up supports and services. This position will be responsible for deploying supportive counseling and skill building.
Principal Responsibilities:
- Conduct program specific screening and evaluation, and formulate diagnosis in accordance with the Diagnostic Statistical Manual (DSM).
- Provide brief case management, service coordination and community liaison services to assure that clients are dispositioned to appropriate levels of care and according to clinical practice standards.
- Provides immediate therapeutic mobile response services to children and youth in Rutland County on a 24/7-hour basis.
- Triage clinical emergencies and implement program specific safety and consultation protocols.
- Utilize best practices and evidenced based practice models where appropriate and adheres to model and program expectations.
- Provide mobile service in community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program. May transport clients to appointments or linkages as appropriate.
- Develop and document, clear clinical formulations, crisis and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant.
- Demonstrate knowledge of internal and external levels of care, referral processes and resources.
- Make clinically appropriate referrals for identified service needs in a timely manner.
- May participate in community-based meetings and conference calls with emergency departments, inpatient units, congregate care providers, other providers to assist in care planning and to address system issues which may impact increased emergency department and inpatient unit utilization of high needs.
- Provide culturally appropriate, competent and individualized linkage plans in accordance with each client’s age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture.
- Work cooperatively with other members of the treatment team.
- Maintain positive and mutually respectful relationships with other human service providers, agencies, health care vendors, and the community.
- Maintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes.
- Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor.
- Attend required staff, supervision, trainings and in-service meetings.
- Maintain compliance with confidentiality policies and procedures at all times.
Qualifications and Competencies: ·
- Master’s Degree in clinical psychology, social work or related field is required. ·
- Experience working in crisis, emergency room, or community-based services preferred.
- QMHP designation preferred, not required.
- Obtain Allied Health Privileges or status as employee of RRMC after hire.
- Demonstrates knowledge of psychopathology, crisis theory and other behavioral health concerns and primary modalities of psychotherapeutic intervention.
- Experience in crisis intervention services, risk management, assessment of suicide potential and integrated treatment for co-occurring disorders preferred.
- Ability to join with clients and families in distress and work with them in disposition planning.
- Must be willing to provide services in clients' homes and community settings.
- Must be available to work shifts as assigned including overnights and weekends.
- Possess excellent communication and organizational skills.
- Ability to function autonomously in stressful situations and make appropriate assessments in coordination with on-call physician ·
- Must have the ability to utilize computer-supported collaborative work software, and have demonstrated proficiency with Microsoft Office Applications and Electronic Medical Record software.
- Knowledge of maintaining and executing confidential information using HIPPA standards.
- Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont. ·
- Successful completion of a criminal history background check after hire.
