Clinical Case Manager

SUMMARY
Provide therapeutic interventions and case management services to youth who are mentally or emotionally disturbed and their families. Interventions include individual, group, and family therapy as well as goal setting and monitoring and de-escalation techniques. Case management includes compliance with documentation requirements including monitoring of files. Serve as liaison for clients and their families with internal and external resource agencies.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Client Case Support
·       Attends all Clinical Staffing meetings to keep abreast of all client treatment goals and progress.
 
·       Makes referrals for outside testing and evaluation including psychological, neuropsychological, psychosexual, ASD, sleep-study evaluations and any others determined in clinical staffing.
 
·       Facilitates treatment team to coordinating discharge and aftercare planning.
 
·       Coordinates and facilitates treatment and discharge planning meetings.
 
·       Informs youth, family, and ongoing treatment providers of additional services and resources offered through Fairfax County and initiates referrals, as needed. 
 
·       Provide case management services to all youth at Leland House through coordination of meetings, oversight of mandated CSA documentation completion, supporting parents and families in using county resources to apply for Medicaid and documenting contact/correspondence with all family members and professionals for each client.
 
·       Ensure documentation (i.e. IFSP-EZ form, team based planning request form, CSA consent, family parental co-payment forms, copies of guardian paystubs, CON, CANS, etc) is accurate and complete per CSA standards and meets appropriate deadlines.
 
·       Attends court, FAPT and other collateral meetings as needed unless the therapist is required.
 
Clinical Duties
·       Respond to crisis experienced by youth and families while involving members of the family’s team.
 
·       Provide direct clinical service to Leland youth to include at least three individual therapy session per workweek (Monday through Sunday) and one group therapy sessions per workweek.
 
·       Develop, revise, and implement Individualized Treatment Plans, which identify individual needs, strengths, goals and objectives and that best meet the needs of the client.
 
·       Conduct family therapy sessions once per week. Sessions must be in person no less than once per four-week period; other sessions may be conducted via telephone. Session times may require nights or weekends.
 
·       Interprets psychiatric treatment and compassionately communicates it to youth's family to help to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
 
·       Establish and maintain positive professional relationships with legal guardians, social workers, probation officers, families and referral sources to advocate and support our clients.
 
·       Ensure appropriate attention to crisis management issues by promptly responding to emergencies and potentially unsafe situations with youth by providing therapeutic interventions for clients in crisis, to include the use of approved verbal and physical intervention techniques, and Agency behavior management practices while on-site or on-call.
 
·       Actively support the team as a member of the Leland House Treatment Team through weekly team meetings, therapeutic mentoring, coaching and guidance on individual treatment issues and appropriate intervention strategies as well as providing feedback to the team on group issues. Quality Assurance
·       Works with the Director to establish required standards for and perform quality checks on all documentation.
 
·       Participates in the on-call duties and rotation.
 
·       Participate in all required and voluntary in-service trainings and programs.
 
Other duties as assigned.
