Care Coordination Program Administrator

DESCRIPTION


The Care Coordination Program Administrator is committed to improving health outcomes and the quality of life for chronically ill and high risk individuals served at Betty Hardwick Center. This positions works with an interdisciplinary team to coordinate the medical, psychosocial, emotional, therapeutic, and recovery support needs of individuals, using approaches that are culturally-sensitive and trauma informed in order to facilitate wellness and recovery of the whole person. This position provides oversight of the Care Coordination Program to include direct supervision of the Care Coordination Team, development and review of a risk stratification process to assign level of services, evaluation of outcomes, community outreach to establish care coordination agreements/MOUs with key organizations, providing training to Care Coordination Team staff, and collaboration with BHC staff on practice improvement.


QUALIFICATIONS


This position seeks applicants with Bachelor and/or Masters degrees in social services and/or health care and at least 3 years of administrative and supervisory experience. Experience with care coordination or case management is a plus. Must demonstrate leadership and communication skills, possess excellent organization skills with ability to keep accurate records, and have experience with health information systems and strong computer skills. Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Travel and the use of a personal vehicle are may be necessary. Must have a valid Texas driver’s license and be an insurable driver through the Center’s insurance carrier. Directly supervised by the Chief Operating Officer.
