Registered Nurse – Intermediate Care Facilities
Are you a compassionate Registered Nurse who enjoys building relationships and providing person-centered care? Valley Community Services Board (VCSB) is seeking an RN to provide nursing services to individuals with intellectual and developmental disabilities in two residential group homes.
In this role, you’ll work closely with clients, Direct Support Professionals, physicians, psychiatrists, and other members of the support team to promote health, safety, and quality of life. You’ll provide ongoing health assessments, oversee medications and medical supplies, and help ensure clients receive the care they need to thrive in their homes and community.

Starting pay of $68,325–$123,992 per year, dependent upon education and experience.

What You'll Do
Provide comprehensive nursing care in collaboration with psychiatrists, primary care physicians, Direct Support Professionals, and other members of each client’s support team.
Assess clients’ physical health status at least quarterly and identify changes that may require additional care or intervention.
Review daily progress notes and documentation related to medications, medical appointments, and medical procedures.
Oversee medication administration, including the inventory, storage, acquisition, and appropriate use of medications, supplies, and medical equipment.
Provide medication administration training, guidance, and ongoing support to ICF team members.
Build and maintain effective, collaborative relationships with coworkers, healthcare providers, and other external partners involved in client care.
Help promote continuity of care by communicating health needs and coordinating with members of each client’s healthcare and support team.
