School-Based Child Mental Health Case Manager

Valley Community Services Board is seeking a School-Based Child Mental Health Case Manager to join our team. In this role, you'll provide intensive case management services to children and adolescents experiencing serious emotional disturbance (SED) and/or at-risk behaviors in school and community settings.

You'll work closely with children and adolescents, their families, school staff and administrators, and VCSB clinicians to coordinate services and support each child's individual needs. Starting pay of $47,618.83 - $66,378.00 per year, dependent upon education and experience.

What You'll Do

As a School-Based Child Mental Health Case Manager, you'll help children and families navigate behavioral health services while providing support across school, home, and community settings. Responsibilities include:

Provide intensive case management services and support to children and adolescents experiencing serious emotional disturbance (SED) and/or at-risk behaviors.

Work collaboratively with children, adolescents, families, school staff, and administrators to support adjustment and functioning at home, in school, and in the community.

Coordinate services and resources with child and family clinicians, school-based outpatient clinicians, and other members of the child's support team.

Help children, adolescents, and their families navigate crisis stabilization and emergency intervention services when needed.

Provide crisis stabilization and emergency intervention support as required, including carrying an agency-issued cell phone for crisis response.

Develop, implement, and complete Individual Service Plan (ISP) reviews for assigned clients in accordance with agency standards and Medicaid requirements.

Maintain accurate and timely documentation of services and client contacts in accordance with agency and regulatory requirements.

Advocate for children and families and help connect them with appropriate community resources and supports.

Maintain confidentiality and uphold client rights in accordance with applicable DBHDS, DMAS, HIPAA, and agency requirements.
