The role
LPNs on our team do real clinical work — not just observe. You'll carry a patient load, make judgment calls, and be a key link between patients, CNAs, and the supervising RN or physician. If you're looking for a role where your skills are actually used, this is it.
What you'll do
- Administer medications and treatments within LPN scope of practice.- Monitor vital signs, symptoms, and changes in patient condition — and document all of it accurately and on time.- Perform nursing assessments and clinical procedures authorized under your license.- Provide wound care, injections, and other prescribed treatments.- Report significant changes in patient status promptly to the RN, physician, or appropriate provider.- Help with admissions, transfers, and discharges.- Participate in building and updating individualized care plans.- Direct CNAs and support staff where state law and facility policy allow.- Respond to emergencies calmly and competently.- Follow infection-control, medication-safety, HIPAA, OSHA, and facility protocols.- Maintain patient dignity, privacy, and confidentiality at all times.- Work closely with RNs, physicians, CNAs, therapists, and the broader care team.
About you
- Active, unencumbered LPN license in the state of practice — required.- Graduate of an accredited practical nursing program.- Current CPR/BLS certification.- Solid understanding of nursing principles, pharmacology, and clinical documentation.- Able to stay organized and composed in a fast-moving clinical environment.- Strong communicator — with patients, families, and colleagues alike.
Nice to have
- Prior experience in long-term care, skilled nursing, home health, or a similar setting.- Familiarity with electronic health records (EHR).- IV certification where permitted by state law.
