Case Manager (RN) – Full Time Days
New Haven, CT | Onsite | 40 Hours/Week
Yale New Haven Health is seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care across the continuum, ensuring high-quality, efficient, and patient-centered outcomes. The Case Manager collaborates with interdisciplinary teams to manage care plans, facilitate discharge planning, optimize length of stay, and connect patients with appropriate post-acute and community resources.
Key Responsibilities:
- Coordinate and oversee patient care plans from admission through discharge
- Collaborate with physicians, nurses, and healthcare teams to ensure appropriate care and resource utilization
- Develop and implement safe discharge plans and care transitions
- Assist with utilization management, documentation, and compliance requirements
- Advocate for patients and families while connecting them with community and support resources
Qualifications:
- Bachelor’s degree in a clinically related field required
- Active Connecticut RN license required
- Minimum 3 years of acute care clinical nursing experience required
- Case management and/or utilization management experience preferred
- Strong communication, assessment, and care coordination skills
