Salary Range: $53,500 to $61,800 (commensurate with experience)

The County is currently accepting applications for the position of Public Health Care Manager I/II - CMARC. This position provides care coordination for Medicaid eligible children ages 0 to 5 years old and their families; assesses and develops care plans based on need of the child and family; implements health education, nutrition, and psycho-social interventions; provides developmental screenings of children, care, and referral activities; and conducts visits with the child in a variety of settings which include the home, community, office and/or medical office.

Minimum Qualifications & Requirements
Requires considerable knowledge of social work principles, techniques, and practices and their application to specific casework, group work, and community problems; knowledge of psychosocial, socioeconomic, and behavioral problems and their treatment; knowledge of governmental and private organizations and community resources; and skill in establishing rapport with clients and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client’s problems.
Minimum qualifications include a Bachelor's Degree in Social Work or related Human Services degree; or Bachelor’s Degree in Nursing which includes a Public Health Nursing rotation and one year of professional nursing experience, preferably in Public Health; or graduation from a school of professional nursing and two years of nursing experience, preferably with one year of experience in Public Health; or an equivalent combination of education and experience. Degree/Coursework must be from an appropriately accredited institution. Depending on degree a current RN license with the North Carolina Board of Nursing may be required; valid driver’s license required.
